Did this for a friend. It is a sub for a 69' Camaro. Thought I would share it here cause I personally know a number of cars that need this done. Hope I put it in the right location. Please move if needed to.
Here is how I did this. Doesn't mean it can't be done another way, or that I did it right.

1. Mark your cuts,
2. Cut.
3. Make patch.
4. Weld.
5. Grind.
6. Weld again till everything is closed.
7. Open up new bushing hole.
8. I sand blasted to match the rest of the frame since it is getting powder coated next.

Here's a template I had saved that can be printed out to use for the holes. (You may have to resize the image when printing)

How did you determine location for the big hole. I made a template from the good side, flipped it over to the repaired side, but it all doesn't seem to line up the same way like it should. It's like the bracket is different from one side to the other.
Wish I could find a drawing with dimensions and angles of the big hole in relation to the small hole.

I saw that drawing. I think you sent it to me before, but it looks like those dimensions are to the "small"hole on the bushing mounts and not the "big" hole. I still have the small holes, but need to drill the big hole, but can't find any layout telling me where exactly the two holes are in relation to each other. If I get it in the wrong location, it won't be fun putting the subframe in again after it's powdercoated. I know you mentioned 1/8" toleratnce, but seems there should be a drawing somewhere from GM that shows how they located these two holes when they made the bracket.

I guess if the big hole is even close to being in the right place, there would be enough "slop" in the cage nut to line up the bushing bolt, maybe...

Just found a diagram in the Chassis Service Manual in Frame section 2-4. Shows dimensions to the big hole measured off the frame centerline and off of some of the other holes in the front and rear subframes. Hadn't seen this before, might be some good info for others repairing their subframe.
With these dimensions, hole locations should be more accurate than making a template from one side, flipping it over, and using it for the other side.
