The bracket from the firewall to the fender skirt has (3) locations for bolt/washer on each side, for bolting to the skirt. I bought a front end hardware kit from ricks and I was given only (2) bolt/cupwasher sets for each side. Should I have gotten (3) sets for each side or does the bottom hole get a different type bolt/washer.

Nope, The bolt and cup washers are to hold the inner fender up into the fender itself in the middle of the inner wheel well.
The lower 3 bolts should have captive washers to go into the bracket.
