I would pull the wheel off, and then stick a lug in the hole & see if it goes past the flange of the wheel. Too long of lug nut maybe? Maybe you have a metric lug nut and it screws on, it seems, normally, but won't stay torqued.

Wheel Mfg.s have a torque spec for their wheels, and on aluminum, its recomended to check at install, 50 miles and 100 miles.... due to the heat cycle. All Mfg.s are different, so check with them.

With the too deep of a run from a lug nut you might try the thick split lock washers like I'm running to keep the wheel from being a little loose and jiggling the lugs back out. There might even be just a little too much play between lug and wheel causing them to back out.
