Here is what I come up with:

Decode for body number: 154894

69-12437 = Coupe.
LOS = Built in Van Nuys California.
721 = Standard Medium Green Bucket Seats.
57-57 = Car Color is Fathom Green

The OE should have one more digit. It should be two numbers and then the E. The -349 should have a letter instead of a -.

Hello Ron...

Your tag decodes as 67ss350camaro stated, but the DE is actually 10E (5th week of October, 1968).

The -349 is correct for your car... and is often refered to as the "LA work order number". It was somehow used to track the builds through the plant. It won't tell you anything about options.

I saw your car when it was in PA.... nice purchase.
