Passenger side of trans should be stamped vertically, a string of characters, i.e., P____.
Remember on clutch disc replacement, material is important. If you get another aftermarket disc, it will be either semi-metallic or metallic and the same deal will happen again. So you'll have to drive it more often.

Armed with the info you have, visit one of the member's site here and decipher the info.
Measure with a ruler, the shaft diameter below the input shaft. They came in two sizes, 7/8 & 1 inch. Being its a 69, I would imagine it to be a 1 inch shaft.
The only correct way to determine model(?) of Muncie is to remove the side cover and count teeth. This would determine the model(?) for sure.
