I have always heard (and read) that body filler should only be applied over bare, prepped metal. In reality I have seen lots of folks overlap or otherwise apply filler to primed areas after a little scuff. Any comments about how acceptable this is?

If its a catalyzed product it is acceptable. In fact Toyota(and some other OEM's) require any body repairs to be primed with epoxy BEFORE applying filler to maintain the corrosion warranty.
