small hole in the frame the shape of a spade

Does anyone have or seen a hole in the frame like this? what can cause this, tire rub? and how to go about fixing it? the same side up front of the frame has one also but none at all on the drivers side.

Fix.. turn the heat up on the mig, that will burn back any thin bad metal, then build up the holes and grind sand off.

Looks like a bit of wear in the rub marks too, I would ne checking thickness /strength there to.
