Take this for what it's worth, and some here may call me a hack, but this is what I did and it's working great so far. I had leftover pinion gear shims (don't recall the thickness, but i think it was only a few thousandths), and I wrapped it around the shaft inside of the plastic piece. It gave me just enough interference that when I tightened the screws it stays in position.

FWIW I had same issue and got the replacement plastic sleeve kit and put those in. Problem solved.

Confirm visor holder is not cracked from previous over tightening. Replacements are available.
It is a common problem, do you have a source or link to the sleeves you purchased?
 
This is a few links I found to dealers we are familiar with. The first one is supposed to be tried and true fit.

I've always found beer cans to be quite useful for such hacks.
 
I had similar problems with my visors, even with new tips. After trying some mechanical fixes, I resorted to internet "fixes" like very lightly sanding the tips then wiping the tips and receiver with alcohol for a clean "grippy" surface, which also didn't work.

The solution I found was heat shrink tubing. I had a whole pack in different sizes and colors, found the one closest in diameter, cut it to length (just a touch long to accommodate for shrink) and applied heat. I did two layers and got a great result. Now they stay in place and only move when I need them to.

Thanks for the tip. Heat shrink over the post then insert into plastic retainer did the trick.
